The lack of quality learning materials in the teaching and learning process was the main factor that initiated this study on the construction of a more in-depth Self-Learning Module (SLM). The multiple benefits derived from the SLM as an effective learning medium led to the decision in selecting the SLM as a tool deserving further investigation. This study, which extracts learning material of form four Physics, incorporates the design and evaluation of three Daya dan Gerakan SLM; i.e. Daya Graviti, Keseimbangan Daya and Kerja, Kuasa, Tenaga Keupayaan, Tenaga Kinetik dan Keabadian Tenaga. This study has also utilized the Model Teras-Cabang (Shaharom, 1994). This learning module was assessed at two levels; i.e. the pilot study and the formative evaluation. The latter was conducted in two phases; i.e. One-to-One Formative Evaluation (OFE) and Small- Group Formative Evaluation (SGFE). Both formative evaluations were carried out across two independent variables which are teaching duration and gender. Four Physics teachers were involved in the OFE and another 10 Physics teachers participated in the SGFE. The research instrument utilized was a set of the Soal Selidik Kesesuaian Modul Pengajaran Kendiri (MPK) (Guru) (Shaharom, 1994). The findings of the study were analyzed using descriptive statistics and displayed the mean, mean deviation as well as maximum and minimum scores. The research indicated that SLM design is very appropriate at both levels of evaluation; i.e. the OFE and SGFE. In general, the use of the SLM had offered positive implications to the pupils, teachers and curriculum designers and even for the researcher herself.
